AMAZING Adult Day Health offers a continuum of services and supports for elders, including those with complex medical or rehabilitative needs. Our services help those we serve feel their best and be as active as they would like to be. Our caring staff work with each Member to create an individualized Plan of Care that fits their needs and interests. For one adult, this may mean help managing an illness such as diabetes. For another, it may mean social activities, help with medications and meals tailored to their special dietary needs. Depending on each person’s unique needs, the care plan may include services such as nursing care, social activities, healthy, delicious meals, and transportation to and from their local center.
Nursing and Medical Oversight
Nurses monitor the daily health of our Members so they can feel their best and be as active and independent as possible.

- Daily care for illnesses such as diabetes, hypertension and COPD
- Administering daily medications
- Monitoring blood pressure and blood sugar
- Wound care
Our nurses provide caring support to Members with medical challenges and are available to assist with any need that may arise.
Case Management
Our experienced Case Coordinators manage all of a Member’s services to ensure that their plan of care is being followed and their medical, social and other needs are being met.

- Coordinating medical and rehabilitative services
- Monitoring nutrition
- Supporting Members in choosing activities that match their interests
- Helping Members access community resources related to health, housing, financial management, citizenship, etc.
Recreation and Social Activities
Adults who come to our centers can choose from a wide range of group activities that reflect their interests and cultural heritage and help them to make friends.
At Amazing Adult Day Health centers, elders and other adults with medical or rehabilitative needs can stay physically and socially active in a vibrant community center environment, where they benefit from stimulating activities, community events and socializing with their fellow Members.
Adults who come to our centers can choose from a wide range of group activities matching their interests and cultural heritage and make lifelong friends.
Activities vary by location and each Member’s individualized plan of care and may include:
- Art and music
- Exercise classes
- Holiday celebrations
- Community outings
- Crafts
- Computer learning
- Yoga
- Enrichment classes (English, health, etc.)

- Transportation to and from the center each day and to doctor’s appointments
- Independent living skill development, such as self care and managing personal affairs
- Our caring staff, including our experienced Case Coordinators who coordinate services, are here to help with any need that arises to ensure our Members have all the supports they need.
Healthy Meals and Snacks
To help Members feel their best each day, our centers provide healthy, delicious meals and snacks.

In coordination with medical staff, we offer special menus that address dietary needs that elders and other adults with medical needs may have as a result of illnesses such as diabetes and hypertension.
Because each of our locations operate as a culturally-based community center, meals also reflect Members’ cultural heritage so they have a taste of their home country each day. Members can enjoy a tasty meal while taking time to relax and socialize with their friends at the center.
Rehabilitation
Our therapists provide physical, occupational and speech therapy to help Members keep or regain skills such as mobility and language.
Many of the adults who come to our centers have rehabilitative needs as a result of strokes, fall-related injuries or other challenges. Our licensed therapists provide speech, occupational and physical therapy to help Members regain and maintain skills such as:
- Language
- Cognitive skills
- Mobility
- Strength
- Motor skills
Rehabilitation services help elders and other adults regain their ability to function as independently as possible while preventing the need for higher levels of care or hospital readmission. The goal of our rehabilitation services is to help adults who have experienced a challenge such as a stroke or fall return to an independent way of life so they can care for themselves and be as active as possible.
Behavioral Health Services
To help Members deal with behavioral health challenges, we offer a wide range of therapy services including counseling and music and art therapy.
As adults age, they often find it difficult to find support for behavioral health challenges or to access the services they need. This can lead to a decline in self-care and general well-being. To help Members deal with behavioral health challenges, our licensed therapists provide a wide range of therapeutic services including counseling and music and art therapy.
In coordination with medical and case management staff, therapists help the adults we serve deal with a wide range of behavioral challenges including depression, anxiety and grief following loss of a loved one. The goal of our behavioral health services is to help Members manage their mental and emotional health so they can continue living as fully and independently as possible.
Transportation
Our centers provide convenient transportation to and from our centers in wheelchair-accessible vehicles.
For elders and other adults with medical or rehabilitative needs, independence often means being able to go where they need to go without having to rely on family and friends. Our Adult Day Health centers provide convenient transportation in wheelchair-accessible vehicles. This allows Members to be as active in the community as possible without having to worry about having a safe, secure ride. Our staff provide drop-off and pick-up services to and from our centers each day as well as to:
- Medical appointments
- Community activities
- Shopping and other errands
