Caregiver Support & Education
We understand that it can be hard to find the balance between caring for a loved one on top of caring for yourself. Being a caregiver can be a full-time job full of new challenges, concerns, or stress. That is why we not only make sure that the your loved one is taken care of, but that you are, too. We will guide you throughout the process.
The caring staff of at our Adult Day Health centers partner with individuals in need of services, their family members, physicians and significant people in their lives to make joining our centers a smooth process. Our staff is here to help you through the entire process. Whether you need help with paperwork or have questions about the insurance process, we’re here to help each step of the way.
We’ll Walk You Through the Process
To get started, simply call or visit your local center to speak with our staff and schedule a tour. A knowledgeable Case Coordinator will work with you to complete eligibility paperwork and schedule an assessment to help us understand the individual’s needs. We then create an individualized Plan of Care outlining the Member’s needs and which services will benefit him or her.
Members must have a comprehensive medical exam from their physician before joining our centers. Your Case Coordinator can work with the doctor’s office to get exam results and any other health information needed to begin the process.
Insurance
We accept various forms of insurance, including Medicaid. We will also work with the funding source to make sure all needed screenings and paperwork are completed.
